2012 Annual Ball Tickets Application Form

Please fill in the form below in order to apply for tickets for the Annual Ball.  Also, be sure to click on the submit button at the end! 

Please ensure that you have read all the info on the "Ball Info" page, and ensure that you have transfered the full amount for ALL the tickets applied for, BEFORE you fill in the form.  If you do not know the names of your guests or indeed, their perferred meal choice, leave the box(s) blank, but be sure to email us ASAP when you can confirm their names and/or meal choices ("cut off" date 20th Feb).  You will also need the Bank Reference Number for the form.  The St Patricks Society Bank details are as follows:
